As a new patient, you will receive an in depth evaluation with a Doctor of Physical Therapy which will include functional movement assessments, strength testing, range of motion testing, and much more! You and your therapist will discuss your concerns and create a plan that will help you improve your pain and allow you to move better. At AZPI, we understand that every person has different goals and we strive to create a plan that will get you back to living your best life.

We are open for physical therapy Monday-Thursday from 8AM-6PM, and Fridays from 8AM-12PM. Hours for classes vary based on class times, and personal training is scheduled by appointment only.

You do not need insurance to be treated at AZPI. We have cash pay options and offer one of the lowest cash pay rates in the area if you do not have insurance or choose not to go through insurance.
Depending on your insurance provider, you may or may not need a referral. It is important to check with your insurance company if you are unsure. AZPI does not require a referral or prescription since Arizona is a direct access state. If you plan to pay out of pocket, you do not need a doctor’s referral.
We recommend filling all new patient paperwork out online prior to your arrival if possible. This will ensure that you have the full amount of time for your physical therapy evaluation and that valuable time is not wasted filling out paperwork.
Wear comfortable loose-fitting clothing that allows for ease of movement. If you have an injury to a lower extremity that will require examination and/or manual therapy, please bring or wear shorts to your visit. You’ll need to bring a photo ID, your insurance card, a prescription from your doctor (if applicable) to your initial evaluation visit. These documents will be used to verify your identity and ensure a smooth billing process. You will not need these documents during your future visits.
